*1	NOTE: To qualify for the member rate, you must be a member of Japanese Society for Skull Base Surgery.

*	Allied Health Professional: Including healthcare administrators, technicians, assistants, nurses, office support staff, medical students, interns and other health professionals.
*	Resident/Fellow, Student/Allied Health Professionals: Please download the certificate format and submit the signed certificate to the registration desk before the congress.
*	Accompanying Person is spouse ONLY. He/She is not allowed to enter conference rooms.

Payment Method

Registration Confirmation/Receipt

The confirmation and receipt can be printed out through My LaCool account page when you complete the payment.

*	The confirmation can be printed as many times as you wish until the end of congress.
*	The receipt can be printed only once. Please inquire the Registration Desk if you failed to print.

Cancellation Policy

Cancellation must be made to the Registration Desk via email.
The amount of refunds depends on the date of cancellation:

?	Cancellation received by March 31, 2016: 50% of the total amount
?	Cancellation received after March 31, 2016: No refund
